Whether you want to speak with a Polk HealthCare Plan representative to learn more about the plan, take advantage of free health services and screenings provided by the Polk County Department of Health or view upcoming diabetes management and healthy living classes, you can find us in your community by checking the calendar.
Polk HealthCare Plan case managers are located at various places throughout Polk County, Florida. You may not have to travel far to meet with a case manager who can help you determine your eligibility, apply for the plan or help you connect with other community resources. Call Polk HealthCare Plan customer service at (863) 533-1111 for help finding the case manager nearest to you.
