Polk HealthCare Plan case managers are located at various places throughout Polk County, Florida. You may not have to travel far to meet with a case manager who can help you determine your eligibility, apply for the plan or help you connect with other community resources. Call Polk HealthCare Plan customer service at (863) 533-1111 for help finding the case manager nearest to you.
