You can complete a new application online or call Customer Service to request a renewal link be sent to you via email to avoid reentering all your information. You may also request phone or in-office renewal.
Polk HealthCare Plan members may only use Labcorp for their bloodwork and other lab needs.
Plan members may not see a Specialty Care provider without a referral from their Primary Care Provider. If you need to see a Specialist, please discuss with your PCP to obtain a referral.

No, appointments help to save you time, but are not necessary. Walk-ins are welcome 8 a.m. to 4 p.m., Mon.-Fri. on regular business days at the Indigent Health Care administrative office located at 2135 Marshall Edwards Dr., Bartow.

Yes, if the child does not qualify for Medicaid and does not currently have any other health coverage, they may qualify for this coverage.
If you currently have any other health coverage or insurance, Medicaid, Medicaid Share of Cost or Medicare, you are not eligible; however, if you are awaiting enrollment into Medicaid or Medicare you may be eligible. Call customer service at (863) 533-1111.
