What are my copays?

Where can I find my provider information?

Provider information can be found on the front page of your Polk HealthCare Plan Membership card (blue card), listed next to PCP.

How do PHP Members renew their membership?

You can complete a new application online or call Customer Service to request a renewal link be sent to you via email to avoid reentering all your information. You may also request phone or in-office renewal.

Where can I get my lab work done at no out-of-pocket cost to me?

Polk HealthCare Plan members may only use Labcorp for their bloodwork and other lab needs.
